Expanding Access to Care, Strengthening Nursing Education

��Ĵ�ý and Albany Medical Center have partnered to create the Albany Medical Center Transitional Care Clinic in Collaboration with ��Ĵ�ý, a new clinic designed to expand access to care while enhancing hands-on learning opportunities for Maria nursing students.

Located in Albany Medical Center, the clinic represents a shared commitment to improving community health and educating compassionate, skilled nursing professionals.

Transformative Learning, Compassionate Care

“This clinic is an example of collective genius—people with different expertise coming together for the good of the community and our students,” said Dr. Lynn Ortale, President of ��Ĵ�ý. “It truly reflects our mercy mission in action—providing compassionate, holistic care while preparing students to be ready for real-world healthcare challenges.”

This clinic is made possible through generous philanthropic support from Albany County, the Innovation Partnership (a collaboration between Albany County and MVP Health Care), the Renaissance Corporation, and other dedicated community partners.

“We are committed to providing our patients with as many resources as possible at every point in their journey,” emphasized Jason Mouzakes, MD, executive vice president and Albany Medical Center Hospital general director. “This great partnership with ��Ĵ�ý strengthens our ability to do that.”

Under the supervision of faculty from ��Ĵ�ý’s Andrea Lewis Siek School of Nursing and in partnership with Albany Medical College residents, nursing students gain invaluable, real-world clinical experience serving patients in need.

Services Provided
The clinic offers services designed to improve health outcomes and help patients recently discharged from the hospital or emergency department transition smoothly between levels of care:
- Primary and preventive care
- Chronic disease management
- Transitional care after hospital discharge
- Reducing avoidable emergency room visits
- Preventing hospital readmissions
- Addressing social factors including food, housing, and transportation
“Our care focuses on prevention, chronic disease management, and transitional care—helping patients until they connect with a primary care provider,” explained Jessica McNally, Director of Nursing at ��Ĵ�ý.
Clinic Hours
By referral:
- Tuesdays & Thursdays: 4:00pm – 7:00pm
